On 15th March 2010, a door on this wagon fell open after leaving Crewe Basford Hall on 6T22 - the 02:36 engineers' train to Lea Junction. Significant damage to infrastructure en-route was inflicted by the door, with damage to platforms at Birmingham International and Watford Junction, as well as signal KR3334 in Kilsby Tunnel on the WCML in Northamptonshire, which knocked out the signalling in the local area. After a quick investigation of the signal post, the last train through was instructed to stop and examine the train, where it was discovered the 11th wagon in the train had a lot of damage to a door that had fallen open. This was the death sentence for the wagon, it being carded at Wembley where it stayed until being moved for disposal in 2017.
